Working alongside a range of other health professionals, supporting people to remain in their own home. The successful candidate will be driven by a passion to deliver exceptional services to people with health, disability or injury related needs in their own homes.
This role includes the following:
Completing assessmentsDeveloping and reviewing goal focused service plans based on individual client needsEducate the client/family and support teamsStaff trainingClinical oversight and some clinical tasks
Essential for this role:
Registered Health Professional with a current NZ practising certificate - Registered Nurse, Occupational Therapist or PhysiotherapistExperience and knowledge or the primary/community health sectorCurrent, full New Zealand driver's licenceCompetent computer skills - a wide range of applications are used in this roleIdeally one to three years' post graduate experience (preferably community based)
